As eMinja mentioned, it's sort of a part of Westeros folklore. King Jaehaerys used to say "madness and greatness are two sides of the same coin." Hence the saying, when a Targaryen child is born (out of incest) the gods would flip a coin. Speculation says this applies to Joffrey as well, and his coin obviously landed on madness.
